I would say that, nowadays they'll be pretty close to the actual
marked size:

Goodrich is the only company I've found that acutally has the real tire
specs shown. Their tires tend to run under the rating sizes by .2" for
the Mud-Terrain T/A® KM and .5" for the AT T/A KO

I'd expect to see the other brands similar. Keep in mind that as the
tire wears it will get considerably smaller than the sidewall rating.
